INSTRUCTIONS: Submit this application with a current curriculum vitae, recent photograph, two letters of recommendation from diplomates of the ACCC, copy of your state license, and six censored copies of IME's, x-ray reports, or claims reviews; of those six , two must be claim reviews. An agreement contract will be forwarded to you that will require your signature and notarization.

Membership Classifications
 | PROVISIONAL 2 YEARS - 2 YEAR MAXIMUM - for those matriculating through the ABCC requirements. |
| BOARD ELIGIBLE - 3 year maximum - in route to becoming a diplomate - must be matriculated in or have successfully completed the ABCC - approved program. |
| DIPLOMATE - A full member who has been certified by the American Board of Chiropractic Consultants. |
Professional Prestige

| ACCC is the only national organization representing the individual chiropractic provider of QA/UR activities. |
